Coinsurance Options:

Inside the United States and Canada

  • Plan A: After you pay the deductible, the program pays 80% of the 1st $5,000 of eligible expenses, then 100% to the selected Medical Maximum.
  • Plan B: After you pay the deductible, the program pays 75% of eligible expenses to the selected Medical Maximum.

Outside the United States and Canada

    • Plan E: After you pay the deductible, the program pays 100% to the selected Medical Maximum.
    • Plan F: After you pay the deductible, the program pays 80% of eligible expenses to the selected Medical Maximum.

Pre-Certification Requirements 

The following expenses must be pre-certified by contacting Seven Corners Assist before the expense is incurred (for an emergency hospital admission, pre-certification must be made within 48 hours after admission or as soon as reasonably possible).

  • Inpatient Care
  • Any Surgery or Surgical Procedure
  • Computerized Tomography (CAT/CT Scan) & Magnetic Resonance Imaging (MRI)

Failure to follow pre-certification requirements could result in a 50% penalty.
